Independence physician reopens under new value-based care model

Dr. Allison Medlin is reopening her practice under the CenterWell Senior Primary Care brand, bringing CenterWell’s personalized, care-team approach to the local 65+ community in Independence, Mo. The remodeled center, located at 300 N. Osage Street and Truman Road, is where Dr. Medlin previously practiced, and gives seniors the option to again see a trusted provider closer to home.

CenterWell Senior Primary Care is one of the fastest-growing senior-focused, value-based care providers in the country. Together with its sister brand Conviva Care Center, CenterWell Senior Primary Care delivers care to seniors in more than 220 centers across 11 states, with plans to open 30-50 new centers per year through 2025. CenterWell accepts patients with many different Medicare Advantage health plans.

CenterWell’s value-based care model is an integration of doctors, nurses, social workers, behavioral specialists and other professionals, specially trained to meet the needs of the senior population. This will be the second CenterWell Senior Primary Care center to offer the care team approach in Independence. The first, located at E. 39th Street, continues its rapid growth since opening five years ago, serving more than 2,000 patients. This center recently completed an expansion to accommodate its growing numbers, adding three more exam rooms.

CenterWell Senior Primary Care provides a wide range of extended services focused on caring for seniors’ particular needs:

  • Longer appointment times – up to 40 minutes or longer, as needed
  • Special chairs that quickly convert to examination tables
  • Wheelchair-enabled weight scales and other features to aid those with mobility issues
  • On-site pharmacies and laboratories
  • Activity centers offering education and socialization attached to many centers
  • Convenient, one-stop experiences for the senior patient
  • Resources for transportation, financial assistance or housing needs
