LOUISVILLE, Ky. (December 12, 2025) – Humana and Trinity Health have reached an agreement with Mount Carmel Health System in Ohio, Saint Joseph Health in Indiana, and Trinity Health in Michigan to gain in-network access to quality, affordable care for Medicare Advantage members.
We are committed to helping our members achieve their best health through coordinated, holistic care. Our relationship with these providers within the Trinity Health System is important to deliver on that commitment and support preventive care for seniors.
Members with questions or in need of assistance finding an in-network provider can contact Humana’s toll-free customer support line listed on the back of their insurance card.

Q: Why were both parties able to reach an agreement?

Q: How should Humana members select an in-network provider at Mount Carmel Health System in Ohio, Saint Joseph Health in Indiana, and Trinity Health in Michigan?
A: Humana Medicare Advantage members can view in-network providers in their plan’s provider directory. The in-network status of the Trinity Health providers will take up to 60-days to update in the system. Humana members will be able to select Trinity primary care providers as their PCP, once updated in Humana’s online provider directory.
