Two centers up and running – one more to go!
The first two of three new senior-focused primary care centers are open in Charlotte, North Carolina, offering a personalized, care-team approach to healthcare for the city’s 65+ community. CenterWell Senior Primary Care University City, 8727 JW Clay Boulevard, and CenterWell Wilkinson, 3512 Wilkinson Boulevard, have officially opened their doors and are seeing patients.
CenterWell Senior Primary Care is one of the fastest-growing senior-focused, value-based care providers in the country. Together with its sister brand Conviva Care Center, CenterWell delivers care to seniors in more than 220 centers across 11 states, with plans to open 30-50 new centers per year through 2025.
An additional center will open in Charlotte this year. CenterWell Wendover is expected to open this summer at 901 North Wendover Road.

CenterWell’s team of medical professionals work to treat all the factors affecting patients’ health. The range of extended services and features at CenterWell Senior Primary Care that focus entirely on caring for seniors’ particular needs include:
“We have a unique care team approach here,” said Dr. Brian Walker, who cares for patients at CenterWell University City. “In all the years I’ve been practicing, CenterWell is the one place where I believe we really put patients in the center. Our care coaches follow our patients if they go to the hospital. We follow their condition and treatment and ensure we schedule appointments once they leave, so we can provide better care and keep them out of the hospital.”
Describing the work of additional care team members onsite who collaborate to ensure whole-person health for every person, Dr. Walker said, “We meet in the mornings to discuss our patients. We have social workers who help manage behavioral health issues. We also have referral specialists and pharmacists who can help manage patients’ care and ensure they have the right medications to keep patients healthy.”
CenterWell Senior Primary Care works with the community to ensure seniors have the resources needed to live more active, healthy lives. As part of CenterWell’s arrival and commitment to improving the lives of seniors in the community, the company gave $20,000 to The Shepherd’s Center of Charlotte, a nonprofit that provides a network of support for the senior community to increase social connection, expand access to critical resources and combat the negative effects of isolation.
For some seniors the struggle with limited socialization and loneliness can be challenging. In addition to partnerships with local organizations, many CenterWell Senior Primary Care centers include activity centers that are open to everyone in the community, not just patients. Centers host movie and popcorn events, bingo and educational classes on Medicare, nutrition and healthy living.
Humana’s Primary Care Organization, which operates both the CenterWell Senior Primary Care and Conviva Care Center brands, serves more than 240,000 members enrolled in many different Medicare Advantage health plans, as well as some patients who have Original Medicare.
