CenterWell Senior Primary Care Centers Open Across the Dallas-Fort Worth Area in 2023

CenterWell Senior Primary Care debuts in north Texas, celebrating the grand opening for the first of 10 centers to offer seniors a personalized, care-team approach to health care.

CenterWell North Buckner, located in Dallas at 3312 N. Buckner Blvd., Suite 213, began seeing patients after Christmas. CenterWell is one of the fastest-growing senior-focused, value-based care providers in the country. Together with its sister brand Conviva Care Center, CenterWell Senior Primary Care delivers care to seniors in 235 centers across 11 states, with plans to open 30-50 new centers per year through 2025.

“The Dallas-Fort Worth senior population is growing, and our focus on underserved communities brings a new level of access to quality, coordinated care to these seniors,” said Sue Lamb, CenterWell Senior Primary Care Market Vice President for Dallas-Fort Worth. “We believe it’s important to understand the community’s needs so that we may better serve them. Everything we do is about treating the patient as a whole person. It’s our honor and privilege to take care of seniors as if they’re our family.”

As part of CenterWell’s arrival and commitment to improving the lives of seniors in the community, the company currently partners with Catholic Charities, the Tarrant Area Food Bank, and the North Texas Food Bank, Lamb said.

CenterWell Senior Primary Care is more than a primary care clinic; it’s committed to treating the whole person, which includes caring not only for physical and mental health, but also addressing social and environmental factors that influence health. What that means for patients is on-site teams of doctors, nurses, pharmacists, social workers, behavioral health specialists and referral specialists, providing a wide range of extended services focused on caring for seniors’ particular needs:

  • Longer appointment times – up to 40 minutes or longer, as needed
  • Special chairs that quickly convert to examination tables
  • Wheelchair-enabled weight scales and other features to aid those with mobility issues
  • In-house laboratories and pharmacies in many locations
  • Activity centers offering education and socialization
  • Convenient, one-stop experiences for the senior patient
  • Resources for transportation, financial assistance, nutrition and housing needs

Many CenterWell Senior Primary Care centers are located in underserved areas and work to remove barriers to care by making clinic visits convenient for the community. The physicians and staff operate a value-based care model that focuses on patients’ overall health and well-being.

“I joined CenterWell as a physician, because we look at health care not as a privilege, but as a necessity,” said Dr. Kiona Coleman, a CenterWell provider currently seeing patients at the North Buckner location. “We take a holistic approach to primary care, which means we look at the whole person and everything that affects a senior’s well-being: mental, spiritual, financial and physical.”

CenterWell Senior Primary Care works with the community to ensure seniors have the resources needed to live more active, healthy lives. Throughout the 2022 holiday season, CenterWell staff offered food vouchers for holiday meals and desserts, donated essential items like paper goods and cleaning supplies to area seniors and partnered with Loads of Love to provide laundry supplies. This in addition to partnering with Catholic Charities and the two area food banks serving people needing food assistance, including senior citizens living on fixed incomes.

Nine additional centers join North Buckner in the Dallas-Fort Worth metroplex. CenterWell Seminary and Camp Bowie West in Fort Worth began seeing patients in late 2022. Redbird, Arlington, South Garland, Grand Prairie, North Richland Hills, Richardson and CenterWell Northwest Dallas will open throughout 2023. Details for those grand opening celebrations will be released at a later date.

Humana’s Primary Care Organization, which operates both the CenterWell Senior Primary Care and Conviva Care Center brands, serves members of many different Medicare Advantage health plans, as well as some patients who have Original Medicare.
